The severe anterior extension of the maxillary sinuses restricted posterior implant placement even with the placement of tilted implants, requiring a specially designed implant milled bar to support a full-arch monolithic zirconia superstructure. The patient also required comprehensive occlusal reconstruction for functional stability and esthetics, increasing the complexity of prosthodontic planning and execution.
