Digitally Coordinated Full-Arch Implant Rehabilitation: A Case Study

This case exemplifies the coordinated digital approach to complex full-arch implant rehabilitation.

The main challenge was anatomical and occlusal limitations. The severe anterior extension of the maxillary sinuses restricted posterior implant placement even with the placement of tilted implants, requiring a specially designed implant milled bar to support a full-arch monolithic zirconia superstructure. The patient also required comprehensive occlusal reconstruction for functional stability and esthetics, increasing the complexity of prosthodontic planning and execution.

Professor Sorensen, a leading expert in implant-supported prosthodontics with over 40 years of clinical experience and extensive teaching in both conventional and digital workflows at the University of Washington, provided expert guidance throughout the treatment. Treatment was performed along with Dr. Sarah Haddad, a 3rd-year Graduate Prosthodontics Program Resident. Besmile’s technical team carried out the prosthetic design and fabrication. The successful outstanding treatment with the technical team and clinical team being on two different continents demonstrates the unique advantages of the digital platform workflow. As demonstrated by significantly decreased treatment time by reduction in shipping of lab work back and forth as well as greatly improved 3-D communication.

Throughout the process, the two teams maintained close communication regarding anatomical landmarks, critical design considerations, occlusal reconstruction, and treatment goals. Within a unified digital workflow, key parameters — such as framework accuracy, passive fit and load distribution—were carefully controlled through systematic team collaboration, addressing potential challenges early in the design phase. This approach ensured predictable outcomes and long-term stability for the full-arch prosthesis.

Dr. Sorensen is Professor, Restorative Dentistry Dept; Director of B4T Laboratory; Director of Research, Graduate Prosthodontics Program at the University of Washington. With 40 years of clinical experience in implant-supported prosthodontics, including serving as the Managing Doctor at ClearChoice Portland, he brings great expertise to full-arch implant treatment. For the past 13 years, he has taught University of Washington Graduate Prosthodontics residents in both conventional and digital workflows, All-on-4 surgical-prosthodontic treatment rationale and digital materials and technologies.
